After witnessing a bizarre, traumatic incident involving a patient, Dr. Rose Cotter (Sosie Bacon) starts experiencing frightening occurrences that she can’t explain.   • The film was originally titled “Something’s Wrong With Rose” before being renamed “Smile.”
  • Smile was announced in June 2020. Principal photography began on October 11, 2021, in New Jersey, including in the city of Hoboken, and wrapped on November 24, 2021.
  • Some filming locations included the Murphy Varnish Lofts, Rutgers New Jersey Medical School in Newark and Lewis Morris Park in Morristown.
